Information you provide
Fleet Safety Advisors may receive contact, company, USDOT, operational, and service-request information that you choose to provide through the website form, email, or phone.
How the information is used
We use this information to review and respond to a request, determine whether and how Fleet Safety Advisors may assist, communicate about a proposed or active engagement, and maintain necessary business records.
Google Forms and service providers
The service-request form is hosted by Google. Information submitted through it is processed using Google’s systems and may be stored in Fleet Safety Advisors’ Google Workspace. Other operational service providers may process information when reasonably necessary to run the business or deliver an agreed service.
Sensitive information
Do not use the initial service-request form to send Social Security numbers, driver-license numbers, dates of birth, medical information, drug-and-alcohol testing records, or documents containing sensitive personal data. Jeremy will explain a more appropriate transfer method if records are needed after the initial review.
Retention and safeguards
Fleet Safety Advisors uses reasonable administrative and technical safeguards and limits access to information used for business purposes. Information may be retained as reasonably necessary for an inquiry, engagement, recordkeeping, legal, or regulatory obligation. No method of electronic storage or transmission can be guaranteed completely secure.
